San Francisco City Council Approves $16.2M Increase for Institute on Aging's Community Living Fund Program, Extending Term to 2027

The City Council of San Francisco approved the First Amendment to a grant agreement between the City, through its Department of Disability and Aging Services (DAS), and the Institute on Aging. This amendment increases the grant amount by $16,216,128, bringing the new total to $27,630,868. The original agreement, approved by the Board of Supervisors on June 6, 2023, and entered into on July 1, 2023, was for $11,414,740. The term of the agreement is also extended by two years, now running from July 1, 2023, through June 30, 2027. The grant provides funding for the Community Living Fund Program, which offers case management and services to older adults and adults with disabilities to prevent institutionalization and support independent living. The amendment also clarifies the scope of services to include additional CalAIM services and adds appendices for federal funding consistency. The Community Living Fund Program provides intensive and moderate case management, and purchased services, including subsidies for assisted living facilities and move-related costs.
