The project objectives include maintaining continuity of services for Community Supports and ECM programs through Epic, implementing Epic-based workflows for referrals and care coordination, and training staff. Additionally, it aims to increase utilization of BHS services and reduce ED visits for ECM-enrolled clients by June 2026.
