This resolution approves the second amendment to the grant agreement between Homeless Prenatal Program and the Department of Homelessness and Supportive Housing, extending the term by four years from June 30, 2025, through June 30, 2029, and increasing the total grant amount by $13,826,617 to a new not-to-exceed of $22,627,326. Under this amendment, the SHARE program will continue rapid rehousing services for at least 32 family households annually and adds 28 one-time rental subsidy slots funded through the City’s Safer Families Plan, bringing the total to 60 families in FY 2025-26. Rapid rehousing services include housing-focused case management, housing location and coordination, subsidy administration, landlord liaison, and workforce development. Subsidies range from 12 to 24 months under SHARE, with an annual recertification process, while Safer Families slots provide one-time move-in support for up to five years. The amendment also lowers the case manager ratio from 1:20 to 1:14 and maintains performance benchmarks for move-in rates, length of homelessness, and housing stability outcomes. All funding is sourced from Proposition C Gross Receipts Tax revenues.
