The Board of Supervisors approved a grant agreement with Homebridge, Inc. to deliver Contract Mode In-Home Supportive Services under the IHSS program, enabling eligible older adults and adults with disabilities to receive managed in-home care coordination and service delivery. The four-year agreement runs from July 1, 2025, through June 30, 2029, with an initial not-to-exceed authorization of $39,600,000 for the first year and subsequent year funding to be requested in Spring 2026. DAS issued RFP #1151 in December 2024, appointed Homebridge as the sole provider, and secured Commission approval on March 5, 2025. Under the contract, Homebridge will recruit, train and supervise providers, manage service schedules, coordinate heavy cleaning, personal care, protective supervision and paramedical tasks, and integrate wrap-around supports for approximately 1,100 clients at any time. The agreement sets a $63.61 hourly reimbursement rate, requires EVV compliance, and mandates ongoing quality assurance, reporting and training standards. This ensures continuity of care for recipients unable to hire or supervise independent providers.
