The funds are dedicated to providing supportive services for eligible residents at Berwyn East Place, an 88-unit permanent supportive housing facility for low-income seniors. These services are aimed at individuals who have struggled with homelessness, mental illness, and substance abuse, aligning with HUD's definition of chronic homelessness. The initiative supports an innovative 'triple blend' service model that includes rental subsidies, care coordination, and health and wellness services to improve housing stability and health outcomes for formerly homeless older adults with advanced geriatric conditions. The service contract will run from December 1, 2025, through July 31, 2030.
